What to Review Before Annual Enrollment

Your Medicare plan may be changing. Every year, Medicare Advantage and prescription drug plans can adjust premiums, deductibles, copays, prescription coverage, provider networks and extra benefits. Some plans may reduce their service areas or leave certain regions entirely.

That does not mean you should switch plans. It means that letting your coverage renew without reviewing it could be an expensive mistake.

Medicare Open Enrollment runs October 15 through December 7, and any change you make takes effect January 1. Use that window to review the Medicare plan changes most likely to affect what you pay and where you receive care: your prescriptions, pharmacy, doctors, hospitals, premium, deductible and cost-sharing. Then weigh those changes against how you expect to use the plan next year.

Begin With the Annual Notice of Change

If you have a Medicare Advantage plan or a stand-alone Medicare prescription drug plan, your plan sends an Annual Notice of Change (ANOC) each fall. It explains the changes in coverage, costs, provider networks and service area that take effect in January.

Medicare says this notice should reach you by September 30. If it does not, contact your plan and ask for a copy.

Do not stop at the first page, and do not look only at the premium. A premium can hold steady while other costs and coverage rules shift in ways that matter more.

Will Your Prescriptions Be Covered the Same Way?

Start with every prescription you take. Note the exact drug name, the dosage and how often you refill it.

A drug can stay covered but move to a higher tier, increasing your share of the cost. A plan can require prior approval, limit the quantity covered or require you to try another drug first. A pharmacy that gave you preferred pricing this year may not hold that status next year.

Review each of these questions:

Is each prescription still on the plan’s list of covered drugs?

Has any prescription moved to a different cost tier?

Are there new approval requirements or quantity limits?

Is your preferred pharmacy still in network, and does it still offer preferred pricing?

Would mail order or another participating pharmacy change what you pay?

Do not assume that a drug covered this year carries the same cost or rules next year.

Are Your Doctors and Hospitals Still Participating?

If you have a Medicare Advantage plan, provider access deserves a fresh review. Check your primary care doctor, your specialists, your preferred hospital and any outpatient facility you use regularly.

The annual notice may flag network changes, but it will not answer every question about a particular doctor or facility. Use the plan’s current provider directory, then confirm the relationships that matter with both the provider’s office and the plan. Ask about your specific plan, not simply whether the office takes Medicare.

This matters most if you have a planned procedure, receive ongoing treatment or want to keep access to a particular health system.

What Will You Pay When You Use the Plan?

The monthly premium is only one part of the cost. Review what you may pay when you receive care.

Look at:

The medical and prescription drug deductibles

Primary care and specialist copays

Inpatient hospital and outpatient surgery costs

Diagnostic imaging, laboratory work and therapy costs

Urgent care and emergency room costs

The plan’s maximum out-of-pocket limit for covered medical services

The maximum out-of-pocket limit deserves attention, but it is not a forecast of what you will spend. It is the most the plan can require you to pay during the year for covered Part A and Part B medical services under the plan’s rules. Premiums, prescription drug costs and services the plan does not cover are not included.

If the increase you are seeing comes from an income-related Medicare adjustment rather than your plan, changing plans will not remove it. Read Why Is Medicare Charging Me More After I Retired?

Are Important Benefits Changing?

Many Medicare Advantage plans include benefits such as dental, vision, hearing, transportation, fitness programs and allowances for certain health-related items. These benefits can change from one year to the next.

Review what is covered, the dollar limit, how often you can use the benefit and whether you must use a particular provider or vendor. A benefit headline can sound generous while the fine print makes it far less useful for your situation.

Extra benefits can help you decide between two plans that otherwise meet your needs. They should not outweigh access to your prescriptions, doctors, hospitals and medical care.

Has Anything Changed for You?

Your plan is only one side of the review. Your health and circumstances may have changed too.

Consider whether you have:

Started a new prescription

Received a new diagnosis

Added a specialist

Scheduled a procedure for next year

Changed your preferred hospital or pharmacy

Moved, or expect to spend more time away from home

A plan that looks much the same on paper may no longer fit if the way you use healthcare has changed.

Keeping Your Current Plan May Still Be the Right Decision

Annual Enrollment is an opportunity to review your coverage. It is not a command to change it.

If your prescriptions remain covered appropriately, your providers participate, the costs fit your budget and the plan continues to meet your needs, keeping it may be a sound decision. The purpose of the review is to make that decision deliberately.

Do not begin by asking which plan advertises the most extra benefits. Begin by asking whether your current coverage still fits the care you are likely to need next year.

If your annual review has you reconsidering how you want to receive Medicare, read Medicare Advantage or Medigap: How Do I Decide?

Review the Changes With Someone Who Knows Medicare

Work with the licensed agent or broker who helps you with your Medicare coverage. A useful review begins with your prescriptions, doctors, preferred hospitals, expected healthcare needs and budget. It does not begin and end with the premium or a list of extra benefits.

If you live in Tennessee and do not have a Medicare agent or broker to help you review your coverage, call Kurt Lytle at 615-610-9945 or schedule a conversation.

If you found this helpful, subscribe to IUL.Solutions.
You’ll get new articles about retirement income, Social Security, annuities, long-term care, Medicare, life insurance and the decisions that can shape your retirement.
Subscribe on LinkedIn → Secure Future Life & Annuities Newsletter

Sources

Medicare Annual Notice of Change.
