Your income went down. Then you received a notice saying that your Medicare premiums were going up.

That surprise has a name: IRMAA.

IRMAA stands for Income-Related Monthly Adjustment Amount. It is an additional amount some people pay for Medicare Part B and Part D when their income exceeds certain thresholds.

It is not a separate Medicare plan.

It is not a late-enrollment penalty.

It is not something you can eliminate by shopping for a different Medicare company.

It is an income-based adjustment, and the financial event that triggered it may have happened two years before the higher Medicare bill arrived.

Why Is My Premium Based on Income From Two Years Ago?

Social Security generally determines IRMAA using income reported on your federal tax return from two years earlier.

That means the Medicare premiums you pay this year are generally based on income you reported two tax years ago.

This creates an obvious problem for someone who has recently retired.

The tax return under review may show a full year of wages, bonuses, self-employment income, or other earnings. Your current income may be considerably lower, but the determination may initially be based on the financial life you had before retirement.

That is why IRMAA can feel like a bomb that goes off long after the fuse was lit.

What Income Is Counted?

IRMAA is based on modified adjusted gross income, commonly called MAGI.

For this purpose, MAGI generally includes your adjusted gross income plus tax-exempt interest. Social Security receives the applicable income information from the IRS.

Several financial decisions or events may increase the income shown on a tax return:

Wages or self-employment income

Taxable distributions from traditional IRAs and 401(k)s

Required minimum distributions (RMDs)

Roth conversions

Realized capital gains

Taxable income resulting from the sale of a business or property

Other significant taxable income

These events do not automatically mean someone has made a bad decision. A Roth conversion, business sale, or large retirement-account distribution may be entirely appropriate.

The point is that a decision made for one purpose can affect something else.

A taxable financial move today may affect your federal income taxes now and your Medicare costs two years later.

IRMAA Can Affect Both Part B and Part D

Most people who encounter IRMAA first notice it as an increase in their Medicare Part B premium.

But IRMAA can also apply to Medicare Part D prescription drug coverage.

The Part D adjustment is separate from the premium charged by the insurance company providing your prescription drug coverage. It can also apply when your drug coverage is included in a Medicare Advantage plan that provides prescription drug benefits.

Changing Medicare plans does not remove an income-related adjustment imposed by Medicare.

This is one reason Medicare should not be treated as an entirely separate conversation from retirement income and taxes. The decisions may be made in different offices, but they can still affect one another.

One Higher-Income Year Does Not Necessarily Mean Higher Premiums Forever

IRMAA is recalculated as newer tax information becomes available.

If your income was unusually high for one year and then returned to a lower level, the adjustment may eventually decline or disappear. That does not necessarily happen immediately because of the two-year lookback.

You should also review the income information shown on your IRMAA notice. If Social Security used incorrect or outdated tax information, there may be a process for requesting that the determination be reviewed.

The important point is not to assume that an IRMAA notice is permanent.

It is also not wise to assume that every IRMAA determination can be changed.

What If I Recently Retired?

Retirement may qualify as a work-stoppage life-changing event.

Social Security recognizes certain life-changing events that may allow someone to request a new IRMAA determination when household income has decreased. These can include stopping work, reducing work, marriage, divorce, the death of a spouse, loss of certain pension income, and several other specifically defined events.

The request is generally made through Social Security using Form SSA-44, along with documentation of the qualifying event and the reduction in income.

A large one-time taxable event, by itself, does not necessarily qualify someone for a new determination. The circumstances and the reason income changed matter.

If you receive an IRMAA notice after retiring or experiencing another major life change, read the notice carefully and contact Social Security to determine whether you may request a review.

You can learn more from Social Security’s official guidance on requesting a lower IRMAA.

The Bigger Lesson Is Coordination

IRMAA is not simply a Medicare issue.

It sits at the intersection of Medicare, retirement income, and taxes.

A CPA may evaluate the immediate tax consequences of a Roth conversion or investment gain. An investment professional may evaluate the portfolio decision. A Medicare professional may explain how income affects Part B and Part D costs.

The problem comes when each decision is considered in isolation.

That does not mean Medicare costs should control every financial decision. Paying IRMAA may sometimes be an acceptable consequence of a decision that produces a greater long-term benefit.

It means the cost should be understood before the decision is made.

As discussed in Taxes in Retirement: How to Keep More of Your Retirement Income, the amount of money available to spend in retirement can be affected by more than the income-tax bracket alone.

Before Lighting the Fuse

Before taking a large taxable distribution, completing a Roth conversion, realizing a substantial capital gain, or making another major income decision, ask:

Could this affect my Medicare premiums two years from now?

You may still decide to proceed.

But it is better to make that decision knowing the possible Medicare consequence than to discover IRMAA when the letter arrives.

The IRMAA bomb rarely goes off when the income is received.

It usually goes off two years later, when the Medicare bill arrives.

Start a Medicare conversation.

If you found this helpful, subscribe to IUL.Solutions.

You’ll get new articles about retirement income, Social Security, annuities, long-term care, Medicare, life insurance and the decisions that can shape your retirement.

Subscribe on LinkedIn → Secure Future Life & Annuities Newsletter

Kurtz Lytle is the founder of IUL.Solutions, an independent insurance and retirement income practice based in Nashville, TN.
